Block Staffing for Children and Adults - Often times both children and adults require nursing care that takes more than just a one hour visit to the home.  Our nurses can come to your home for as little as 2 hours to as many as 24 hours per day to provide that care you need. This can be either short-term (for an acute illness) or long-term (for Hospice, disabled, or chronically ill individuals).

RN Case Management – A registered nurse will oversee your plan of care to ensure you are receiving an adequate level of services. The RN will review your treatment plan with you, your family, and your doctor at regular intervals. A RN can also accompany you to your doctor's appointment to ensure your needs are met and your questions are answered in a way that you understand.

IV Therapy- Whether you need IV antibiotics or a longer term treatment, our nurses have the skills to provide these services to you at home.  We are experienced in maintaining IV’s, PICC and central lines (longer term IV lines that are placed by doctors or specially trained nurses), and ports (an implantable device under the skin for IV access placed by a surgeon).

Home Health Aides - These are known as Licensed Nursing Assistants (or LNA’s).  They are licensed by the state of NH and are specially trained to assist clients with activities of daily living (ADL’s), such as bathing, dressing, and transferring.  Some can also assist the nurses with basic wound care and other tasks.

Homemaker Services - If you are in need of someone to prepare meals for you, perform light housekeeping, or offer companionship, a homemaker is right for you.

Medication Management - A nurse can come to your home and help you organize your medications.  Your nurse will coordinate the correct medications you should be taking with your doctor and help you fill a weekly medication planner.  Your nurse will also review side effects and help make you aware of what to look for when you are taking a particular medication.

Diabetes Management - A nurse will help you monitor your blood sugars, and coordinate proper care with your doctor. Our nurses can advise if your blood sugars are running too high or too low and recommend that your medications be adjusted.  Nurses can also draw blood as ordered by your doctor to check your HgbA1C (a test that checks an average blood sugar over the previous 3 months).  If appropriate, she can also instruct in insulin administration.

Respite care – Anyone providing care for a loved one knows how difficult it can be to find time for him or herself. Our caring professionals at Live Free Home Health Care, LLC understand this. We can provide those needed breaks for family members and caregivers, giving you the peace of mind that your loved one remains in good hands. Ask about our half day, full day, vacation, and bed & breakfast plans.

Blood Draws - An experienced nurse can come to your home to draw your blood that has been ordered by your doctor and take it to your local hospital lab for processing.  Our nurses can draw a wide variety of tests from PT/INR (coumadin monitoring) to fasting cholesterol and other tests.

Private Duty Professional Nursing - Often times both children and adults require nursing care that takes more than just a one hour visit to the home.  Our nurses can come to your home for as little as 2 hours to as many as 24 hours per day to provide that care you need. This can be either short-term (for an acute illness) or long-term (for Hospice, disabled, or chronically ill individuals).

Pediatric Skilled Home Care - We offer a wide range of pediatric services.  A newborn wellness check, weight monitoring for babies, or care to a disabled child are just a few examples where a child may require skilled home care.

Chaperoned Physician Visits - It can be difficult to remember everything you wanted to talk to your doctor about when you get into the exam room.  And sometimes you don’t always understand everything your doctor explains to you.  Have you ever wondered if there is help for these situations?  We can have a Registered Nurse accompany you to your physician visit to help you understand your doctor better and to help your doctor understand your feelings as well.  The nurse can help ensure that you are getting the quality of care that you deserve.
