Obtaining Our Home Care Services

The first step in obtaining our home care or home healthcare services is calling our office. As a medical home care agency, a Registered Nurse will perform an initial assessment with the client and/or the family and will develop an individualized plan of care based on the determined needs. This care plan is then shared with the client’s primary care provider. This sets us apart from a non-medical or companion home care agency in that the client's care plan is frequently assessed and updated by Live Free Home Health Care’s Registered Nurse in collaboration with the Primary Care Physician, helping to ensure cohesive care across providers. These services enable the elderly to stay in their own homes while giving their families peace of mind.

Live Free Home Health Care accepts private pay, Medicaid, long term care insurances and we credit card payments. We can also assist clients in exploring financial options to pay for care.
