Services
To help you or your loved one maintain independence and remain safe at home, we can provide home care or home healthcare services from as little as two hours a day to 24/7 around the clock care individualized to the client’s needs.
Each plan of care is individually designed, updated and supervised by a Registered Nurse.
Our private duty services include but are not limited to:
Personal Care
Assisting with bathing, hygiene, and dressing, and supervision of client safety.
Exercise
Assisting with walking and exercise.
Housekeeping
Light housekeeping, making beds and changing linens, laundry and ironing.
Meals
Planning, preparing and cleanup of tasty nutritional meals that encourage appetite, monitoring food expirations and assisting with grocery shopping.
Transportation
Running errands, shopping, picking up prescriptions, transporting to doctor appointments or social engagements, and airport assistance.
Organization
Medication supervision/management, organizing mail, clipping coupons, assisting with client calendar, and arranging appointments.
Companionship
Providing conversation and encouragement, playing mind stimulating games, reading, participating in crafts and personal escorting to outings and events.
Our home health care services include but are not limited to:
Licensed Nursing Assistants
Offers a wide variety of assistance including: personal care, medication reminders, assistance with transfers, use of most in home medical equipment, assistance with range of motion and strengthening exercises.
Skilled Nursing Services
Includes I.V. therapy, diabetes management, blood draws, wound care, medication management, nutritional counseling, and coordination with primary care providers.
Our specialized services include:
Respite Care and Hospice Assistance
Temporary home care to provide family members and caregivers with relief, rest, and peace of mind.
Advanced Health Watch System
A powerful option on hand for Live Free Home Health Care's clients, is our technology-based solution to monitor for falls, emergencies, or problems that may arise at those times when caregivers are not scheduled to be in the client’s home. Alzheimer’s/Dementia Care
Our caregivers are specially trained in Alzheimer’s and Dementia care, gently encouraging someone with Alzheimer’s, using patience, specific activities, diversion and some shared laughter. Learn more about care for someone with Alzheimer’s or Dementia.
In-Home Safety Assessment
As a part of our services, we will walk through a client’s home to assess for simple ways to make living at home safer and more comfortable, whether the concern is to avoid a fall or make living with a particular medical condition a little easier.
